Dehydration is a pervasive challenge in long-term care, accelerating patient decline and causing preventable suffering.
We exist to solve this challenge by delivering physician-led IV protocols that measurably improve patient outcomes and quality of life.
Medical leadership

Kuljit Kapur, MD
Dr. Kuljit Kapur is a board-certified physician and healthcare executive with an MBA, with over 15 years of experience in leading and managing various medical and wellness services, such as nursing homes, hospice, palliative care, cosmetic medspa, and AI innovation. She is currently the Chief Development Officer and Medical Director at Chestnut Health Company, a provider of integrated physical medicine and rehab.
Dr. Kuljit Kapur is also the Chief Medical Officer at Harter Investment Strategies, a firm that invests in innovative and disruptive healthcare technologies and solutions, such as Guidance Airway Solutions, a company that developed the Firstlook LTA device, a novel and minimally invasive approach to airway management, and MTAH, a multi-translating actuation head robot. As part of this role, she also helps develop and manage Luxeskin and Luxe wellness in Naperville, IL, where cosmetic and aesthetic services are offered, along with medical advice for pain, weight loss, anti-aging, and metabolic medicine. Dr. Kuljit Kapur mission is to improve the quality of life and care for patients and clients across the lifespan, using evidence-based, holistic, and compassionate approaches.

Steve Merahn, MD
Steven Merahn, MD, is a veteran physician-executive who provides strategic and clinical advisory services focused on care delivery redesign, system innovation, and person-centered care, with special focus on improving the quality of health and quality of life of targeted populations and communities with complex health-related needs. At Vitaline, Dr. Merahn is responsible for ensuring the basic science rationale and clinical foundation of Vitaline services, policy and protocol development, oversight of our quality and outcomes programs, and advising our facility partners on optimal implementation.
Most recently, the Medical Director for two NY-based non-profit organizations serving over 30,000 children and adults with intellectual and developmental disabilities (IDD), Dr. Merahn has spent the last decade focused on the care of patients with functional and adaptive skills limitations and disability, including autism, mental health, and frailty.
Dr. Merahn is an established thought leader on healthcare transformation and a vocal advocate for health equity for socially disadvantaged communities. He is the author of Care Evolution: Essays on Health Care as a Social Imperative.

Avi Lev
Growing up among healthcare operators and professionals, Avi developed a focused ambition to contribute to the nursing home space. In 2015, he joined Villa Healthcare as its chief marketing officer, working closely with its executive team and developing strong operational expertise.
While in an operational role, Avi was made aware of the dehydration challenges facing patients in SNFs. After encountering a micro model providing patients with proactive IV hydration, he developed the objective of bringing this transformational intervention to facilities across the country. Partnering with an expert medical team and other skilled administrative professionals, Avi has led Vitaline to its current elevated level of both clinical and operational excellence.
Avi lives in Chicago with his wife Tami – a registered nurse and COO at Vitaline – and their growing family.

A healthcare business development and operations professional, Yosef brings 17 years of healthcare experience to the team. His focused, incremental approach to problem solving supports Vitaline’s consistent clinical and operational excellence. Yosef synthesizes strategic insights from the team to improve process and customer experience so Vitaline can consistently deliver clear value for patients, their families and community stakeholders.

Aryeh Polstein
Over his ten years in the healthcare industry, Aryeh has delivered value on executive levels in both sales and operations. Before joining Vitaline, Aryeh served as chief marketing officer for Eden Senior care, a portfolio of nursing homes and senior living facilities across the Midwest. At Vitaline, Aryeh channels his operational prowess expanding Vitaline’s reach as it strives to mitigate dehydration challenges in SNFs and other senior living facilities throughout the country.

Tami Lev
Tami grew up watching her urologist father impact lives as a healthcare professional and dreamed of making a similar impact. After graduating from nursing school, she pursued a specialty in oncology and infusions, where she worked for twelve years in private practice.
When her husband, Avi Lev, founded Vitaline, Tami threw her clinical expertise behind the Vitaline mission. In her role as Chief Operating Officer, she guides the team in setting and surpassing safe, efficient standards in clinical implementation.

Scott Sklar
Scott Sklar is the Vice President of Operations at Vitaline, bringing a rare combination of technical expertise, operational leadership, and a focus on continuous improvement. After a decade as a software engineer, he earned his nursing home administrator license to pursue his passion for advancing care delivery in post-acute settings.
With over 15 years of leadership as an Administrator, RDO, and VP of Operations, Scott has driven innovative initiatives including redesigned call light systems, advanced staffing models, and streamlined medication processes. At Vitaline, he oversees national operations and clinical systems, leveraging his deep SNF and technology background to scale compliant, outcomes-driven solutions that help facilities proactively address hydration and elevate resident care.

April Doremus
April began her career in hospitality sales, where she developed a service-centered, people-first mindset. Seeking more meaningful impact, she transitioned into healthcare over a decade ago, initially leading patient satisfaction initiatives and quickly expanding her role to include employee engagement, marketing, communications, and employee relations.
April has worked with Vitaline since 2023, supporting the organization’s growth through her leading role in Business Development and Account Management. She guides a high-performing team focused on expanding Vitaline’s reach across SNFs and other senior living facilities nationwide, always with a commitment to prioritizing the needs of residents and the facilities that serve them.

Mary Frances Tesher
Mary Frances Tesher is a results-driven Human Resources executive with over 30 years of experience in the healthcare industry. Her expertise in leading, recruiting, and coaching high-performing teams has been instrumental in shaping Vitaline’s approach to employee experience and retention.
With a Bachelor of Science in Industrial & Organizational Psychology from Trinity Christian College and a Master of Arts in Organizational Leadership from Lewis University, Mary Frances is passionate about fostering a supportive and inclusive work environment that empowers employees to thrive.
